Parents do this Assessment for their child.
The Assessment applies to children who are 5 to 11 years old or are in Kindergarten through 5th grade. We recommend that both parents do the Child Assessment together if possible and discuss differences of opinion that may arise. Discussing aspects of your child’s behavior and early development together will help you determine the right answers.
One tie between two different modalities often occurs and is valid. If your results show two ties, you will be prompted to retake the Assessment to break one of the ties. Close collaboration with your child’s other parent often changes scores slightly and eliminates one of the ties.
Instructions: Rate each statement by how much you agree with it: very true, mostly true, somewhat true, or not true.
Important: Some statements may feel like they have two parts. Use the part that feels most true and score that part. If no part feels true, score “not true” for the entire statement.
