It applies to adolescents who are 12 to 18 years old, or who are in 6th through 12th grade.

Read and answer each question based on what feels true most of the time. If you are confused or are stuck on a word, ask a parent to clarify the meaning of the question.

One tie between two different modalities often occurs and is valid. If your results show two ties, you will be prompted to retake the Assessment to break one of the ties. Giving some additional thought to the questions or collaborating with a parent can change your scores slightly and eliminate one of the ties

Instructions: Rate each statement by how much you agree with it: very true, mostly true, somewhat true, or not true.

Important: Some statements may feel like they have two parts. Use the part that feels most true and score that part. If no part feels true, score “not true” for the entire statement.
